We have several options for providing service for Homebound patrons:
- Holds Authorization Form for the Caregiver. Use this form to allow your caregiver to pick up your holds for you.
- Caregiver / Homebound Authorization Form. Use this form to allow your caregiver to have access and manage your library account.
- Dane County Library Service. If you don't have a caregiver to pick up your holds, we can connect you with the Dane County Library Service who will get materials to you!
